What's It Like to Be a Dermatology PA? 100+ Questions Answered

Sometimes the specialty picks you. When I was pre-PA, the only office that returned my shadowing calls was a dermatology practice, and my assumption that derm was "just acne" dissolved fast. Dermatology is unique in that the pathology is visible: you see what's happening rather than inferring it from numbers, and you literally watch patients get better. That instant gratification hooked me.

The path in: shadowing connections, elective rotations in derm (do them at a practice that might hire you, because that's how I ended up with a job offer before graduation), and comfort with a steep first-year learning curve, since PA school typically includes only about two weeks of dermatology.

Dermatology is clinic-schedule work: roughly 8 to 5 in private practice, an actual lunch break most days, and the ability to design your week, including part-time arrangements (two days a week is workable, with surgeries planned before lunch as an overflow buffer).

For patient volume, I see around 35 a day at an established full-time practice, and it was more like the mid-20s while I was building a panel from scratch. And the variety is real! I've seen everyone from infants to a 103-year-old, with skin checks and cancer surveillance, acne, psoriasis, eczema, and a steady stream of "weird spots."

There's plenty of hands-on work: shave and punch biopsies, excisions including skin cancers, cryotherapy, plus cosmetic procedures like Botox, fillers, and peels, depending on the practice.

And if you're squeamish, that's something you can train out of. I nearly fainted at my first punch biopsy as a shadowing student (skin is deeper than you expect), and now I do them all day without a second thought. Rotations will recalibrate your stomach, so don't rule out procedural specialties over a first reaction.

Charting varies wildly by office, from all-day EMR screens to hybrid paper-and-dictation systems where you barely touch a computer. Ask about it when job hunting, because it shapes your daily life more than almost anything.

In the right practice, you get as much as you want. I have full ownership of my own patients, with a collaborating physician reachable and happy to eyeball anything weird, which after five years might mean asking a few times a month.

The first six months are different: exhausting, humbling, and I came home tired every night from all the learning (psoriasis alone, with its many presentations and medications, takes a while, and pediatric skin takes even longer). The real skill isn't memorizing everything. It's knowing what you don't know and asking without shame, with confidence and humility in equal measure.

I'll be honest, there are a few:

• Booking horizons. A busy derm PA can be booked 3-6 months out, which means vacations get planned half a year ahead. Clinic-schedule work rewards planners, while shift work (hospitalist, ER, urgent care) suits a more spontaneous temperament, so know which you are before job hunting.

• - Insurance conversations. Patients want every mole and disliked spot removed, but insurance covers the harmful and malignant, not the cosmetic. Explaining that every day gets old.

• - Draining cysts. They smell, and that's all I'll say about that.

None of it outweighs watching a teenager's acne (and confidence) transform. That's still the best part.
